I would like to know the meaning of this ghazal?

It's a very beautiful Ghazal, lyrics are just awesome... it tells about a person who is in his last days of life... his body is not in the condition to sustain his love... even if things go right now there is no hope left... he says you're there in every thought that I live but who cares if you're there in it or not... the world is over for me, everything is going to end soon... my presence or absence will not make a difference anymore, I should not bother why no one supported me at this time of life...
